- Mechanism of Action (MOA)
- Binds the 30S ribosomal subunit → misreads mRNA → inhibits protein synthesis (bactericidal).
- Black Box Warning
- Nephrotoxicity and neurotoxicity/ototoxicity (usually irreversible); fetal harm in pregnancy.
- Clinical Notes
- Aerobic gram-negatives; synergy with β-lactams for gram-positive endocarditis. Trough monitoring; concentration-dependent killing.
