Drug Class: Aminoglycoside
Mechanism of Action (MOA): Binds the 30S ribosomal subunit → misreads mRNA → inhibits protein synthesis (bactericidal).
Black Box Warning: Nephrotoxicity and ototoxicity (often irreversible), neuromuscular blockade, and fetal harm.
Clinical Notes: Aerobic gram-negatives; synergy with β-lactams for gram-positive endocarditis. Trough monitoring; concentration-dependent killing.
