Activates antithrombin III → inactivates thrombin (IIa) and factor Xa.
Clinical Notes
Monitor aPTT; rapid on/off, reversible with protamine. Watch for HIT (heparin-induced thrombocytopenia). Preferred in pregnancy and renal failure.
